You can easily pay your bill online. Just have your invoice at the ready, so you can reference your unique statement and account numbers. Then we’ll route you to patientnotebook.com/proliance, a secure online site to make a payment.

Going Cashless for Safety

We’ve gone cashless at all our care centers. To ensure our patients and staff remain safe, we’re asking our patients to bring alternative forms of payment, such as a debit card, credit card, cashier’s check, and/or a prepaid gift card (like VISA or Mastercard) that can be found at any local grocery or drug store.

Billing FAQ

No. All services provided in our clinic locations are billed as office visits (Place of Service: Office). This includes:

  • Provider visits
  • Physical Therapy (PT)
  • MRI and other in-clinic services
  • Urgent care / walk-in visits

These services are subject only to your office visit benefits, such as copays, coinsurance, and deductibles based on your insurance plan.

Our urgent care and walk-in services are billed as office visits (Place of Service: Office) because we are not credentialed with payors as a designated urgent care facility.

This means your visit will be processed under your office visit benefits, not urgent care benefits.

We recommend checking with your insurance plan in advance, so you understand how your benefits apply.

You would only receive a separate facility bill if your care takes place in a hospital-based setting, such as:
  • Emergency Room (ER)
  • Inpatient hospital stay
  • Hospital outpatient department
In these cases:
  • We bill for the professional services (provider care)
  • The hospital bills separately for the facility services

If you have a procedure at one of our surgery centers:

  • You will receive a professional bill from your provider
  • You will also receive a separate facility bill from the Ambulatory Surgery Center (ASC)

Your costs depend on where you receive care. Clinic services are billed as office visits, while hospital-based services and surgery centers may involve separate facility charges, which are processed differently by your insurance.
