Grant Eligibility Criteria
Before applying for funding from the Proliance Surgeons Foundation, it’s essential to ensure that your organization meets our grant eligibility criteria. These criteria serve as guidelines to ensure alignment with our mission and focus areas, as well as geographic considerations and organizational affiliations.
- Grantee initiatives should align with one of Proliance Surgeons Foundation’s core focus areas: workforce development, community engagement, social determinants of health, or health equity (see above).
- Grantees are expected to serve communities within Proliance Surgeons Foundation’s operational areas in Washington state, covering Pierce, King, Skagit, Whatcom, and Snohomish counties.
- Organizations with religious affiliations are ineligible for funding.
- Grantee entities must not operate as non-profit foundations.

Greg E. Davis, MD
Greg Davis, MD, is an otolaryngologist (ear, nose and throat surgeon) who sub-specializes in rhinology, the medical and surgical treatment of nose, sinus and skull base disorders. He has received several awards for excellence in patient care including numerous years of “Top Doc” awards from Seattle Met Magazine, Seattle Magazine, as well as the American Academy of Otolaryngology’s “Honors Award” and the “Patient Top Choice” Award from Vitals.com.
Dr. Davis is a Seattle native who attended the University of Washington for his undergraduate education, medical school and surgical residency. He then traveled to Australia to train with world renowned sinus surgeon, Professor PJ Wormald. Following completion of his training, he practiced for 12 years at the University of Washington Medical Center, serving as the Director of Rhinology and Endoscopic Skull Base Surgery and was the Director of the rhinology and endoscopic skull base surgical fellowship. During this time, he cared for patients from around the region with the most complex and challenging sinus and skull base disorders.
Dr. Davis left his academic position at the University of Washington to focus on the most enjoyable aspect of his medical practice: treating patients. Joining private practice in Puyallup allows him to provide the highest quality medical and surgical care in a very convenient environment for patients. He treats his patients with considerable care, respect and kindness from the simplest illness to the most complex.
Dr. Davis is a physician educator and co-director of the world’s largest sinus surgery educational course called the American Rhinologic Society’s Summer Sinus Symposium. He also co-directs the annual Seattle Otology and Advanced Rhinology Course. He is the past president of the Northwest Academy of Otolaryngology. He is also on the Board of Directors for the American Rhinologic Society. He gives frequent lectures both nationally and internationally on improving medical and surgical treatment of sinus disorders. Dr. Davis is married and has two adult children. He enjoys traveling, skiing, scuba diving, hiking, rock climbing, mountaineering and camping.
Charlie A. Peterson, MD

Charlie A. Peterson, MD
Charles A. Peterson II, MD, is a board-certified orthopedic surgeon with subspecialty interest and extensive experience in knee and hip replacement, sports medicine, and shoulder surgery. He has been in practice since 1996.
Dr. Peterson believes it is important to listen to his patients, and fully answer their questions. He takes care to understand how an injury or condition is affecting each patient’s daily life and activities, and craft a a treatment plan specifically suited to that patient.
During his training at the Mayo Clinic and Hospital for Special Surgery (New York), he was a team physician for the New York Mets, New York Giants, the Association of Tennis Professionals, and St. John’s University.
Dr. Peterson also has helped lead Proliance Surgeons, sitting on the Board of Directors from 2004 until 2021, and holding the positions of President and Chairman from 2014 to 2022. He also served as interim CEO from 2017 to 2019, and in 2021.
A lifelong athlete, Dr. Peterson and his wife enjoy all forms of outdoor sports and activities in the Great Northwest. This dovetails nicely with his love of photography.

Justin Crowe, PhD
Justin serves as the Vice President, Human Resources for Proliance Surgeons where he leads the people function in support of Proliance’s 100+ Care Center locations. Justin has over 18 years of experience in healthcare and has served in multiple HR, operations, and divisional executive roles. Prior to joining Proliance, Justin served as the Senior Vice President of Community Partnerships at Providence.
Justin has served on multiple boards, most recently the Seattle | King County Workforce Development Council, and was a GE Administrative Executive Fellow with the Health Management Academy from 2017 – 2019. He is certified as a Senior Professional in Human Resources (SPHR) and has a PhD from the University of Michigan and a Bachelor’s degree from Carleton College. Justin is married with three children and enjoys hiking, mountaineering, and single origin coffee. His passion is empowering employees and community members to discover their unique gifts and create an impact at work and where they live.

Daniel Olsen, CPA
Daniel Olsen is the Financial Controller of Proliance Surgeons, responsible for overseeing the accounting operations in support of Proliance’s 100+ Care Center locations. Daniel has 10 years of accounting experience as a Washington State CPA in healthcare, public accounting, and private equity. His passion lies in enhancing lives and preserving our natural environment. Daniel is a native of the Pacific Northwest and earned his BS in Business Administration and Accounting from the University of Washington-Bothell.

John D. Pryor, MD, is trained in minimally invasive and motion-preserving spine surgery, complex spine surgery, spinal fractures, tumors, and trauma. His professional philosophy is to advise and treat his patients the same as he would his own family. He is committed to providing his patients with all the information available about how to treat their condition. If surgery is necessary, Dr. Pryor is able to provide the latest, state-of-the-art techniques to ensure the best care possible.
He enjoys outdoor activities including fly fishing, cycling, and photography.

Mary K. Thayer, MD
Mary Kate Thayer, MD, is a fellowship-trained orthopedic surgeon who specializes in hand and upper extremity surgery. She cares for both adults and children.
Dr. Thayer received her BA in psychology from the University of Washington and her MD from Saint Louis University School of Medicine where she graduated Alpha Omega Alpha (highest honors). She then returned home to the Northwest and completed her orthopedic surgery residency at the University of Washington. To finalize her training, she relocated to Baltimore, Maryland, and completed a fellowship in hand and upper extremity surgery at the Curtis National Hand Center, one of the top fellowship programs in the country. There, she received world-class training in the treatment of both common and complex disorders of the hand and upper extremity.
Dr. Thayer is published in the topics of hand, wrist, and trauma surgery and has presented much of this research at national and local meetings. In addition, she has a passion for mentoring and education. She has participated in the Perry Outreach Program and the Ruth Jackson Orthopaedic Society’s mentorship program. These programs both share a goal of inspiring young women to become leaders in orthopedic surgery.
Dr. Thayer finds satisfaction in helping patients restore function of their upper extremity to return to work, family, hobbies, and sports. She works directly with patients to develop individualized care plans, both surgical and non-surgical, to achieve these goals. This is achieved through open communication with patients and education about the diagnosis and treatment options. She prides herself on a thorough and thoughtful approach to each patient. She has a special interest in fractures, sports injuries, arthritis, and nerve conditions of the upper extremity.
Dr. Thayer is a local Washingtonian. In her free time, she enjoys spending time with her husband, Lew, and her daughter. She is an avid runner, cooking enthusiast, and Seattle sports fan.

Peter F. Maurice, MD, is a highly specialized otolaryngologist, and fellowship-trained in plastic surgery. He is double board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology.
Dr. Maurice received his medical degree from Georgetown University, in Washington, DC, where he was elected to membership in the Alpha Omega Alpha Honor Medical Society. He completed his residency in otolaryngology–head and neck surgery at Georgetown University Hospital.
Dr. Maurice then was selected for a fellowship in facial plastic and reconstructive surgery. This full-year training program gave him a unique opportunity to study with one of the AAFPRS’s master surgeons in Albuquerque, New Mexico.
At South Seattle Otolaryngology, patients benefit from a team of on-site audiologists and allergy specialists, in addition to a team of otolaryngologists with over 50 years of combined experience.

Misty Sullivan, MBA
Misty Sullivan, currently serving as the Director of Operations at Proliance Rainier Orthopedic Institute, is a dedicated healthcare professional with a wealth of experience spanning over a decade. Her journey in healthcare commenced at Proliance Puget Sound Orthopedics as an Operator and evolved through various roles, including Patient Services Representative, Referral Coordinator, and Authorization Coordinator.
Misty holds a Bachelor of Science degree with an emphasis in Healthcare Systems Management and a Master's degree in Business Administration, both earned from the University of Arkansas Grantham. Additionally, she possesses an Associate's degree in Billing and Coding, showcasing her commitment to mastering different facets of the healthcare industry.
Beyond her professional roles, Misty is a devoted member of Proliance's Diversity, Equity, and Inclusion (DEI) Committee, where she actively contributes to the mission of ensuring that every Proliance employee feels included, respected, and supported. Her passion for operations shines through her leadership as she chairs the referral work group and serves as a valuable member of the Data advisory committee.
Misty's dedication extends to her role as a mother, with one son who is currently a Senior in high school and is set to embark on a Radiology Technician pathway in college starting in the summer of 2024.
In her own words, Misty firmly believes in Proliance's compassionate ethos, seeing it as a caring organization that values its team members and is poised to make a significant impact on the community by advancing health equity. She is especially passionate about fostering leadership skills among employees and facilitating opportunities for their growth and advancement within the organization.
Being one of the largest and most trusted surgical groups in the Pacific Northwest, we have a duty to give back. This foundation is our platform to make a meaningful impact in the healthcare landscape, and we are excited about the possibilities it brings.
