FAQ for Self-Pay Packages
What happens when I purchase a package?
Once you've selected the self-pay package you prefer, you'll proceed to pay for it online using a credit card, much like other online transactions, through our secure portal. Following the completion of your purchase, a representative from the corresponding Care Center location will contact you to arrange your appointment.
There is no payment required at the time of your appointment, as you've already paid online. After your appointment with one of our providers, you'll receive a receipt for your self-pay package purchase.
What is included in my package?
Our self-pay package offers a comprehensive single visit experience. It encompasses a thorough discussion about your injury history, a detailed physical examination, a personalized care plan, and the option for an x-ray if necessary. Please note that this package does not cover follow-up visits or any supplementary testing. However, additional services and follow-up appointments can be arranged separately as required.
Can I use my FSA or HSA to purchase a self-pay package?
Although Health Savings Account (HSA) funds are applicable for a wide range of medical and dental services, they typically cannot be utilized for self-pay packages. We recommend reaching out to your HSA provider directly to clarify whether self-pay services could potentially be eligible for purchase using your individual HSA account.
Can I purchase a self-pay package over the phone?
You are unable to purchase self-pay packages over the phone. They can only be bought online at one of the locations listed in the "Buy a Self-Pay Package" section of this webpage.

How do I use the self-pay packages with insurance?
Our self-pay packages operate independently from insurance coverage. Opting for a self-pay package means that Proliance Surgeons won't submit a claim to your insurance provider. Depending on your insurance plan, choosing a self-pay package might offer a more cost-effective option. With Proliance Surgeons' self-pay packages, you'll have transparency regarding the appointment cost upfront.
Important Note: By selecting a self-pay package through Proliance Surgeons, you forfeit the option to submit that visit to your insurance. Additionally, you retain the right to request Proliance Surgeons to limit the use and/or disclosure of your Protected Health Information (PHI) for treatment, payment, and healthcare operations.

We’re a group of specialists and surgeons with advanced knowledge in our areas of medicine, treating complex care needs for our patients. And if surgery becomes necessary for you, we have performed more than 68,000 surgeries and procedures a year.
Balance Billing Protection Act
In Washington State, you’re safeguarded against surprise medical bills and balance billing. Balance billing occurs when you’re charged the difference between what your insurance covers and the full cost of care from out-of-network providers. This often happens in emergencies or when unexpected out-of-network providers treat you at in-network facilities. However, you’re protected from balance billing for emergency services and certain in-network facility services. Providers cannot force you to waive these protections. Your responsibility is limited to your usual cost-sharing, and your insurance should cover the rest directly.
What does this mean for you?
For patients, the Balanced Billing Protection Act shields you against the financial burdens associated with out-of-network care. It ensures that individuals do not face excessive medical bills simply because they received treatment from a provider outside of their insurance network at an in-network facility. This legislation offers peace of mind by capping the amount patients can be billed for such services, preventing surprise charges that can disrupt their financial stability. Moreover, by establishing a framework for resolving payment disputes between insurers and providers, the act promotes transparency and fairness in healthcare billing practices, allowing patients to focus on their recovery without the added stress of navigating complex billing issues. Overall, the Balanced Billing Protection Act represents a significant step towards ensuring affordable and equitable access to healthcare for all individuals.

No Surprises Act & Good Faith Estimates
At Proliance Surgeons, we believe in providing transparent and accessible healthcare services to all our patients. As part of our commitment to transparency, we want to inform you about the No Surprises Act, which became effective on January 1, 2022, under Section 2799B-6 of the Public Health Service Act.
What does this mean for you?
In accordance with Section 2799B-6 of the Public Health Service Act, healthcare providers and facilities must notify individuals without health plan enrollment or federal healthcare program participation, or those not intending to claim with their coverage, of their right to receive a Good Faith Estimate for healthcare services upon request or during scheduling. This estimate must be provided both verbally and in writing. It’s crucial to note that these estimates are subject to change, and final billing statements may include additional medically necessary services.
